OFF-SITE TRAINING DOCUMENTATION FORM
This form must be completed for all off-site training for which LOSAP credit is requested.  The form must be authorized by the Captain and returned to the LOSAP Coordinator within 30 days of completing the training.

COMMITTEE MEETING DOCUMENTATION FORM
This form must be completed for all committee meetings for which LOSAP credit is requested.  The form must be authorized by the Captain or President and returned to the LOSAP Coordinator within 30 days of completing the meeting.

SPECIAL EVENTS ATTENDANCE FORM
This form must be completed for all special events for which LOSAP credit is requested. The form must be authorized by the Captain and returned to the LOSAP Coordinator within 30 days of attending the special event.

Blue Light Request form

This is the State MVC blue light request form that must be signed by the squad Captain and Boro Mayor or CEO before being submitted to the state for a Blue Light Permit.  Once submitted to the MVC, the MVC Director will issue a ID permit card to the Boro to be counter signed and presented to the member.  Please review the requirements for using a blue light at the following links.

Training Exempt Form
(FAQ)

Please fill out this form and have your Captain or other authorized officer sign.  This is needed to prove active membership in an First Aid squad and will be accepted as payment for most EMT continuing education courses.  Only EMT's may use this form.

CPR CEU Form

To get CEU credit for taking a CPU course, please fill out the following form and mail it to the NJ OEMS.
